The Upper Yough — Sang Run to Friendsville — is the marquee run, a technical, continuous Class IV–V gorge. Below Friendsville, the river eases into a Class II–III lower canyon.

Long before it was a paddler's river, the Youghiogheny watershed was contested borderland. In the era before European contact, Garrett County's Youghiogheny country was territory shared and disputed among the Shawnee, Seneca, and earlier Monongahela peoples. The frontier settlement period of the 1750s through 1770s brought a different kind of history to the wider Yough basin, which witnessed George Washington's first battle.

The river's modern character was forged by industry. Through the late 1800s, its valley became an engine for Garrett County's economy as agriculture, timber, and coal took hold along the banks. The watershed was logged from the 1830s through the 1920s, feeding regional sawmills, logging drives, and downstream lumber operations. The appetite for timber ran so deep that in 1889 the Confluence and Oakland Railroad was laid specifically to reach the forests crowding the river valley. Large-scale logging finally ended after the old-growth stands were exhausted around 1910, state forestry conservation began in 1915, and state forests were established in the 1930s.

A more permanent transformation came in 1944, when the Youghiogheny River Reservoir dam rose for flood control and hydroelectric power, altering the flow that loggers and farmers had known. The river's hydrology had already drawn scientific attention: early USGS surveys in the 1870s, gauging-station work through the 1880s–1910s, and state geological streamflow assessments in the following decades produced the first comprehensive picture of the Youghiogheny's discharge. Later water-pollution-control studies and Clean Water Act assessments took on more than a century of logging, agricultural, and industrial impacts, with restoration and TMDL programs as the current outcomes.

The river's whitewater legacy is more recent but no less defining. In 1985, Precision Rafting and other outfitters based in Friendsville began pioneering commercial Class V trips on the Upper Yough, and the run became the training ground for a generation of world-class paddlers — the birthplace, in effect, of eastern creek boating. Precision Rafting has specialized in the Upper Yough ever since. Meanwhile, the river endures as a working fishery: its cool currents sustain smallmouth bass and a naturally reproducing walleye population. Framed by Swallow Falls State Park and the Deep Creek Lake Recreation Area and carrying a Maryland Scenic River designation, the Maryland Youghiogheny remains prized for both the sport and the solitude it offers. What began as a frontier corridor of coal and timber now flows on as one of the East's most storied rivers.
